Description
American Traveler is hiring an experienced RN Case Manager for an ED-based Case Management position in Chicago, IL, requiring 3 years of acute care case management experience and an active IL RN license.
Details
- Work is primarily in the Emergency Department (ED) of a 205-bed community hospital with a Level II NICU and no trauma designation
- ED has 35 beds serving an underserved population including adults, geriatrics, and occasional pediatric patients with complex social needs such as housing instability and low-income circumstances
- Common presentations include complex social determinants of health cases; patients frequently utilize the ED as urgent care
- Caseload is approximately 20 patients
- Primary assignment is the ED with possible float to inpatient units to cover call-outs
- EMR: Epic; Careport used for referrals
- 12-hour day shifts, 0700–1900, 36 hours per week
- Every 3rd weekend rotation required
- No on-call requirements
- Self-scheduling with manager review and approval; schedule made in 3-month increments
Requirements
- Active IL RN license required
- ASN or higher accepted
- Minimum 3 years of acute care case management experience required, with most recent experience within the last year
- Strong assessment, critical thinking, and interpersonal skills required
- Must be a collaborative team player comfortable navigating insurance plans and locating community resources
- ED RN experience preferred but not required
- ED Case Management experience not required; training will be provided on-site
- Epic EMR experience
- Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the last year
- Candidate DOB and last 4 of SSN required for consideration
Additional Information
- Responsibilities include working with a multidisciplinary team, navigating insurance plans, connecting patients with community resources, and supporting facility initiatives
- Solo ED RN Case Manager role; inpatient case managers, a CM assistant, and 24/7 hospitalists are available for support
- Orientation includes new employee orientation (NEO) plus 3 shifts of department-specific orientation, with 1–2 additional shifts available if needed
- Current focus includes connecting patients who use the ED as urgent care with appropriate community resources and programs
- Scrubs must be eggplant-colored or business casual attire; a white lab coat is required
- Free parking available on-site
- Holiday coverage required for approximately 2 holidays during the contract
- First-time travelers are welcome to apply
- Local candidates are accepted at the same rate; no radius restrictions
- Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months to be eligible
