Description
American Traveler is seeking a Physical Therapist for a home health and hospice assignment in the Southern Black Hills region of South Dakota, serving a medically complex patient population across a wide geographic area.
Details
- Home health and hospice setting serving the Southern Black Hills area, including Hot Springs, Custer, Edgemont, Hill City, Hermosa, and surrounding communities
- Patient population includes medically compromised individuals with CHF, COPD, DM, and cardiac conditions, as well as ortho, neuro, dementia, and general weakness cases
- Patient ages range from early adults to geriatrics
- Monday–Friday schedule; 8-hour day shifts, 40 hours per week
- No weekend rotation and no on-call requirements
- Office is closed on 6 major holidays
- Mileage is reimbursed at the current IRS standard mileage rate
- Charts in Epic EMR
Requirements
- Minimum 1 year of physical therapy experience required
- Bachelor's degree in Physical Therapy required
- SD PT license required if applicable
- Current BLS certification required
- General PT skills required, including evaluation, intervention planning, medical assessment, and care plan development
- 2 professional references dated within the past year required for consideration
- DOB and SSN required for consideration
- Candidates must reside more than 100 miles from the facility to be eligible
Additional Information
- Responsibilities include patient evaluation, development and implementation of care plans, and ongoing medical assessment across home health and hospice settings
- Interdisciplinary team includes RN, LPN, OT, PTA, SLP, aides, MSW, and case managers
- Orientation is approximately 1 week and includes Epic training, shadowing, and documentation process review
- Any color scrubs or business casual attire accepted; no denim or sweats
- Free parking available on site
