Description

American Traveler is seeking a Physical Therapist for a 13-week home health travel assignment in the San Jose, CA area, requiring a minimum of 1 year of experience and an active CA PT license.

Details

• Home health setting covering assigned county territories in the San Jose area, including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, and Los Gatos
• Additional travel outside the primary service area may be required to maintain weekly visit expectations
• Day shift schedule
• 13-week contract
• Weekend coverage may be required
• Work is paid on a per-visit basis, with hours calculated using a point conversion system (routine visits and evals = 1 point/1.33 hrs; OASIS SoC = 1.5 points/2 hrs)
• Rate is all-inclusive, covering drive/travel time, visit time, documentation time, and phone calls to patients and physicians
• Traveler must be willing to see COVID-positive and COVID PUI patients

Requirements

• Minimum 1 year of physical therapy experience required
• Active California PT license required
• BLS certification required
• Valid driver's license, current auto insurance, and current auto registration required
• Annual TB test, annual flu vaccination, MMR, Varicella, HepB, and TDap (within 10 years) required
• Drug screen and physical required for consideration
• License required if applicable

Additional Information

• Responsibilities include conducting home health evaluations, routine visits, re-evaluations, OASIS assessments, and discharge visits across an assigned geographic territory
• Must be willing to travel outside the assigned service area occasionally to maintain weekly visit expectations
• All requested time off (RTO) must be disclosed prior to acceptance; time off requested after the acceptance date will not be approved
• Former employees of the facility are not eligible for this travel assignment
• First-time travelers are not explicitly excluded from consideration
