Description
American Traveler is seeking a licensed Physical Therapist with home health experience for a skilled home care position serving a diverse patient population across Pinellas County, FL.
Details
- Home health setting providing skilled visits to patients in their place of residence
- Patient population includes med/surg, cardiac, ortho, hip/knee/shoulder post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL patients
- Caseload of 6–8 home visits per day (minimum 6 points per day; may increase at manager's discretion)
- Visits are self-scheduled based on the needs of the HomeCare office
- Day shift, 5x8-hour schedule (8:00–17:00)
- 1–2 weekend shifts required per contract
- Equipment used includes gait belt, rolling walker, sit-to-stand lift, sliding boards, soft/hard collars, standard walker, supplemental oxygen, upper extremity prosthetics, and wheelchair
- Documentation systems include BayCare Forms, BayCare Scheduling, and ACE Timesheet; laptop, MiFi, and portable printer provided
- Covers Medicare, Medicare Advantage, and commercial insurance patients
Requirements
- Minimum 1 year of physical therapy experience required; home health experience required
- Current BLS certification required
- Valid state PT license required (license required if applicable)
- OASIS training preferred
- Valid driver's license and personal vehicle required for home visits
- COVID vaccination required, or exemption form on file
- Flu shot required or declination on file; mandatory mask wearing at all times while on the job if declining
- Proficiency in assessment, cardiopulmonary/circulatory care, documentation, ortho, neuro, home exercise programs, patient/family education, safety and injury prevention, medications, and billing
- Experience with or ability to use standard home health equipment (gait belt, walkers, prosthetics, wheelchair assessment, etc.)

- Responsibilities include skilled home visits, patient and family education, home exercise program development, and documentation
- Any solid color scrubs required
- 2 professional references from a supervisor within the past 2 years required for consideration
