Description
American Traveler is hiring an experienced RN Case Manager for an inpatient care management position requiring a BSN, CCM certification, and recent Level 1 or comparable Level 2 Trauma experience.
Details
- Inpatient hospital setting managing a daily caseload across multiple units
- Patients present with a variety of care needs requiring post-discharge planning including home health and inpatient rehab
- Schedule is Monday through Friday, 8:00 AM to 4:40 PM
- No weekend or on-call requirements
- Documents and places orders within Epic EMR
Requirements
- Active NH or Compact RN license required
- BSN required
- Current BLS certification required
- Certified Case Manager (CCM) credential required
- Minimum 3 years of inpatient clinical experience required
- Minimum 1 year of care management experience required
- Recent Level 1 or comparable Level 2 Trauma experience within the last 2 years required
- Epic EMR experience required
- Two manager references required for consideration
Additional Information
- Oversees daily caseload on inpatient units, completing initial assessments and facilitating individualized care plans in collaboration with patients, families, caregivers, and the healthcare team
- Participates in interdisciplinary rounds
- Communicates consistently with medical teams and patients regarding post-discharge care needs, including home health and inpatient rehab referrals
- Interacts with post-acute care referral sources to establish discharge plans and place orders for home health, DME, and nutritional needs
- Candidates must reside more than 75 miles from the facility to be eligible — local candidates are not accepted
