Description
American Traveler is seeking a licensed Physical Therapist with home health experience for a traveling PT Home Health position serving Medicare and commercial insurance patients across a multi-county territory in Florida.
Details
- Home health setting serving skilled home care patients covered under Medicare, Medicare Advantage, and commercial insurance
- Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL cases
- Caseload of 6–8 home visits per day with a minimum of 6 points per day (may increase at manager's discretion)
- Day shift schedule, 5x8 hours (8:00–17:00)
- 1–2 weekend shifts required per contract
- Equipment provided includes a laptop, MiFi for internet access, and a portable printer
- Documentation systems used: BayCare Forms, BayCare Scheduling, and Ace Timesheet
Requirements
- Minimum 1 year of physical therapy experience required; home health experience required
- Current BLS certification required
- Valid state PT license required (license required if applicable)
- Valid driver's license and personal vehicle required to conduct home visits
- OASIS training preferred
- Proficiency with assessment, cardiopulmonary/circulatory evaluation, ortho, neuro, home exercise program development, patient and family education, safety and injury prevention, billing, and documentation
- Experience with adaptive equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
- COVID vaccination or approved exemption required
- Flu shot or signed declination required; mask must be worn at all times while on the job if flu vaccine is declined
Additional Information
- Responsibilities include skilled home visits, patient and family education, safety and injury prevention, signs and symptom recognition of physical/sexual/psychological abuse, and billing documentation
- Scrub color requirement: any solid color
- Local candidates are accepted for this position
- 2 professional references from a supervisor within the past 2 years required for consideration
