Description
American Traveler is seeking a licensed Physical Therapist with home health experience for a skilled home care position serving a diverse patient population across the Tampa, FL area.
Details
- Skilled home health setting serving Medicare, Medicare Advantage, and commercial insurance patients
- Patient population includes med/surg, cardiac, ortho, hip/knee/shoulder replacements, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL patients
- Home visit caseload of 6–8 patients per day, with a minimum of 6 points per day (may increase at manager's discretion)
- Day shift schedule, 5x8-hour shifts (8:00–17:00)
- Visits and hours are self-scheduled based on the needs of the home care office
- Weekend coverage required approximately 1–2 weekends per contract
- Equipment provided includes a laptop, MiFi device for internet access, and a portable printer
- Documentation systems used: BayCare Forms, BayCare Scheduling, and ACE Timesheet
Requirements
- Minimum 1 year of physical therapy experience required; home health experience required
- OASIS training preferred
- Current BLS certification required
- Active state PT license required (license required if applicable)
- Valid driver's license and personal vehicle required to conduct home visits
- Proficiency in assessment, cardiopulmonary/circulatory care, documentation, ortho, and neuro required
- Experience with home exercise program development, patient/family education, safety and injury prevention, and billing required
- Experience with adaptive equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
- COVID vaccination required or exemption form on file
- Flu shot or signed declination required; mandatory mask wearing at all times while on the job if declining the flu shot

- Responsibilities include skilled home visits, patient and family education, documentation, and care coordination across a diverse patient population
- Any solid color scrubs are acceptable
- 2 professional references from a supervisor within the past 2 years required for consideration
