Description

American Traveler is seeking a Home Health RN with Homecare Homebase EMR experience and at least 1 year of home health nursing experience for a 13-week contract.

Details

• Home health nursing services providing routine visits, discharges, ROC, recertification, and reassessment visits
• Teammates complete 6 patient visits per day, with a requirement of 30 visit equivalents per week
• Manages a caseload of up to 25-30 patients through case/episode management
• Patient population includes chronic disease, wound care, IV therapy, pleurx, and wound vac cases
• Travel throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas, averaging 60-80 miles per day
• Uses EPIC Rover (view only), Focura for uploading discharge instructions and wound photos, and a company-provided phone for documentation and communication
• Documents in Homecare Homebase (HCHB) EMR
• Day shift schedule, 8 hours, 0800-1700, 40 hours per week
• No required weekend rotation unless interested
• On-call not required but may be available
• Schedules patient visits the day prior to the planned appointment

Requirements

• Active RN license (NC or Compact) required
• BLS certification required
• Valid driver's license and proof of insurance required
• Minimum 1 year of home health nursing experience required
• Homecare Homebase (HCHB) EMR experience required with no exceptions
• Skilled in IV therapy, pleurx, wound vac, wound care, patient assessment, and disease teaching
• Experience with OASIS documentation
• Case management experience overseeing 25-30 patients at once
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN

• Schedule cycle repeats every 13 weeks
• No company car provided; mileage is reimbursed weekly
• Cancelled appointments do not count toward required visit equivalents
• First-time travelers will be considered if experienced in home health
• Candidates must live at least 50 miles outside of Albemarle, NC to be eligible
• Previous employees must be separated from any AHSE facility for at least 1 year after a permanent position before returning
