Description
American Traveler is hiring an experienced RN Case Manager for a 13-week acute inpatient Case Management position requiring at least 1 year of acute care nursing experience and knowledge of health care reimbursement systems.
Details
- Acute inpatient hospital setting focused on care coordination and discharge planning
- Collaborates with hospital-based physicians, nursing staff, utilization review, and other healthcare team members
- Manages transitions of care including Home Health, DME, IV Infusion, SNF Sub-Acute, and Acute Rehab referrals
- Works with community providers and ambulatory case managers to ensure safe discharges and prevent readmissions
- Day shift: 8:30 AM – 5:00 PM, 8-hour shifts
- 40 hours per week
- 13-week contract
- EMR: Epic
Requirements
- Active VA or Compact RN license required; all licenses and certifications must be active at time of consideration — pending licensure is not accepted
- Associate's Degree in Nursing (ADN) required at minimum
- Minimum 1 year of experience in acute medical/surgical, ED, or critical care nursing
- Experience using an electronic medical record (EMR) system required
- Knowledge of health care reimbursement systems including Medicare Advantage, commercial payers, Medicaid, and CMS regulatory rules
- Last 4 digits of SSN and date of birth required for consideration
Additional Information
- Completes initial face-to-face assessments for all admitted patients within 24 hours to identify discharge needs
- Documents and communicates assessment outcomes and transition plans in accordance with discharge planning policies and regulatory requirements
- Coordinates referrals with in-network vendors for post-acute levels of care
- Local candidates are welcome to apply
- Candidates who have worked on assignment at this facility for two cumulative years (with lapses of less than 3 months) must take a 90-day break before being eligible for a new assignment
