Description

American Traveler is seeking a licensed Speech-Language Pathologist (SLP) with home health experience for a full-time, 13-week travel assignment in a home health setting in Morristown, NJ.

Details

• Home health setting with travel to patient homes and facilities throughout the service area
• Monday through Friday, 8am–4pm schedule; no weekends
• 40 hours per week, 8 hours per day
• May be required to float to a nearby sister facility as needed
• Uses Epic for charting
• No on-call required

Requirements

• Master's degree from an accredited Speech-Language Pathology program
• Minimum 1 year of experience in an acute, subacute, or home care setting
• Home health experience required
• Active NJ Speech-Language Pathology license required (license required if applicable)
• BLS certification
• Must reside more than 50 miles from the facility to be eligible
• Valid driver's license and personal vehicle required for home visits
• 2 professional references required for consideration

Additional Information

• Conduct patient assessments to determine appropriateness for home care, including baseline evaluation of swallowing, communication, and cognitive abilities
• Collaborate with patients, families, physicians, and interdisciplinary team members to develop and revise individualized plans of care
• Provide instruction in communication, safe swallowing techniques, and cognitive training as indicated
• Implement and evaluate evidence-based treatment programs aimed at restoring function and achieving maximum functional status
• Manage case load and ensure compliance with home care regulations and insurance reimbursement guidelines
• Participate in team and interdisciplinary team (IDT) meetings
• Provide discharge planning and referrals to appropriate resources
• Assumes responsibility for orientation of new employees and cross-training as requested
• Travelers may remain on assignment for up to one calendar year from the initial start date; a 30–60 day break is required before beginning a new assignment at this facility
