Description
American Traveler is seeking a Physical Therapist for a Home Health Outpatient position in the Medford, OR area, with Epic/OASIS experience required and openness to travelers new to home health.
Details
- Home health setting covering Medford, OR and surrounding areas
- 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points is the standard
- 5x8-hour day shifts (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available
- No weekend or on-call requirements
- Charting is done in Epic and OASIS
- Travel between facilities within the state is expected; refusal to float may result in assignment termination
- Occasional patient visits may require driving up to 50+ miles, though scheduling is designed to minimize drive time
- Mileage reimbursement is provided at the IRS standard rate
Requirements
- Current BLS certification required
- Minimum 2 years of physical therapy experience required
- Epic EMR experience required
- OASIS experience required
- Must have reliable personal transportation
- Active PT license required if applicable
- Primary COVID-19 vaccine series plus one booster required
- At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
- SSN and DOB required for consideration
Additional Information
- Open to candidates without prior home health experience who are interested in working in the home health setting
- First-time travelers are not eligible for this position
- Local candidates within a 50–75 mile radius are accepted at the same rate
- Former permanent staff must wait at least 3 months before returning as a traveler and cannot live within 50 miles of the submitting facility
- RTO requests should be confirmed with the hiring manager prior to submission; minimal RTO is preferred
- Any color scrubs are acceptable
- Parking is free at the base location
