Description
American Traveler is seeking a Physical Therapist with a minimum of 3 years of experience and a strong background in skilled nursing facility care for a 13-week travel assignment.

- Patient population includes residents requiring physical therapy to maximize functional abilities and independence in activities of daily living

Requirements
- Minimum 3 years of physical therapy experience required
- Skilled nursing facility (SNF) experience required
- Bachelor's degree required; Master's or Doctorate preferred
- Active PT license in the applicable state required; license required if applicable
- BLS/BCLS certification required
- Must be able to lift up to 50 lbs independently; lifts over 50 lbs require a two-person assist
- Strong verbal, written, and documentation communication skills
- Proficiency in Microsoft Office
- Ability to work collaboratively within an interdisciplinary team
Additional Information
- Responsibilities include resident evaluation, development of individualized plans of care with short- and long-term goals, and ongoing monitoring and modification of treatment programs
- Provides consultation to referring physicians and participates in pertinent team meetings
- Documents all evaluative and therapeutic interactions in the Electronic Medical Record in compliance with Medicare, Medicaid, and other reimbursement regulations
- Supervises Physical Therapist Assistants in accordance with state mandates and professional standards
- Provides mentorship to new staff during orientation and delivers in-service education to nursing and other departments
- Assists in the development of new rehabilitation programs and services
- Maintains clean and organized office and clinical areas in accordance with infection control procedures
