Description

American Traveler is seeking an experienced RN for a Home Health Field RN position in Leander, TX, requiring HCHB and OASIS experience with the ability to hit the ground running in a fast-paced, community-based care environment.

Details

• Home health field setting serving patients across a wide geographic service area in central Texas
• Caseload of 4–6 patients per day with a target of 30 points per week (equivalent to 40 hrs/week)
• Patient population includes geriatric and diverse home-based patients requiring skilled nursing care
• Case types include in-home patient care, interdisciplinary care coordination, and plan of care development
• EMR: Homecare Homebase (HCHB) and OASIS — experience with both is required; charting must be completed at the patient's home
• Schedule is Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
• Weekend availability required as needed
• On-call rotation required, including approximately one night of call per week (details discussed at interview)
• Travel up to 50 miles from home base is required to cover the assigned territory

Requirements

• Active TX RN license or compact RN license valid in Texas — must be in hand at time of consideration (pending licenses not accepted)
• Current BLS certification required
• Minimum 1 year of home health experience required; minimum 2 years of overall clinical experience as an RN required
• HCHB and OASIS experience required; 1 year of experience with both is preferred
• Ability to travel up to 50 miles from the assigned home base

• Responsibilities include performing comprehensive patient assessments, developing and overseeing plans of care, and coordinating with interdisciplinary teams and certifying providers
• Non-smokers strongly preferred, as many patients have COPD and are sensitive to cigarette smoke
• Scrubs in any color are the dress code
• Local candidates are accepted; candidates must reside within a 50-mile radius of the home base location
