Description
American Traveler is seeking an experienced RN for a Care Coordinator position focused on cardiac and heart transplant patient management, requiring a CA RN license and ambulatory care experience.
Details
- Ambulatory care setting within a large academic medical center
- Care Coordinator unit focused on heart transplant and heart failure patients
- Patient population includes heart transplant recipients and patients with heart failure
- Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection
- Day shift schedule: 4x10-hour shifts, 7:30 AM – 6:00 PM
- Floating to other facilities within the same health system is required
Requirements
- Active CA RN license required
- Current AHA BLS certification required
- Minimum 1 year of ambulatory care nursing experience required
- Cardiac nursing experience with management of heart failure and/or heart transplant recipients required
- Experience with Phoenix EMR preferred
- Experience managing other organ transplant recipients preferred
- Working knowledge of immunosuppressive therapies preferred
