Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and OASIS proficiency for a 13-week contract based in the Salinas/Santa Cruz, CA area.
Details
- Home health setting, beginning in the Salinas area with potential reassignment to Santa Cruz based on patient census
- Day shift schedule, 8 hours per day (7:00 AM – 3:00 PM), 5 days per week
- Caseload of up to 5–6 patients per day with a standard productivity expectation of 30 points
- 1:1 patient ratio with an average of up to 6 visits per day
- Weekends may be required as needed
- No on-call requirements
- EMR: Home Health MD
- Mileage reimbursement provided at the IRS standard rate
Requirements
- Minimum 1 year of home health PT experience required
- OASIS experience required
- Current CA PT license required if applicable
- Current BLS certification required
- Must have reliable personal transportation
- Ability to travel up to 50+ miles for patient visits on occasion
Additional Information
- Scheduling system groups patient visits to minimize drive time and windshield time
- Documentation tools will be provided by the facility and must be returned at the end of the assignment
- Timely communication and documentation are expected throughout the assignment
- Business casual attire or scrubs are both acceptable
- No parking costs at the office location
- First-time travelers will not be considered for this position
- Local candidates are accepted
- 2 professional references from a supervisor within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
- All time-off or scheduling requests must be submitted at the time of initial consideration
