Description

American Traveler is seeking a licensed Physical Therapist with Home Health experience and OASIS proficiency for a 13-week travel assignment in Alameda or Contra Costa County, CA.

Details

Home health setting serving patients in Alameda or Contra Costa County, CA
Day shift schedule, 5x8-hour shifts (9:00 AM – 5:00 PM)
Weekends required on an as-needed basis
No on-call requirement
Uses Kinnser/WellSky EMR; training provided

Requirements

Minimum 1 year of recent home health PT experience required
OASIS experience required
Active CA PT license required; license must be in hand at time of consideration
Current BLS certification required

Additional Information

Responsibilities include physical therapy evaluation and treatment of patients in a home health setting across Alameda or Contra Costa County
2 professional references from a supervisor within the past 2 years required for consideration
Full 7-year work history with all gaps explained required for consideration
All time-off and scheduling requests must be submitted at the time of initial consideration
License required if applicable
