Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a contract position providing skilled home visits across cardiopulmonary, neuro, ortho, geriatric, and oncology patient populations in the Dunedin, FL area.

Details

• Home health setting with skilled home visits
• Patient population includes cardiopulmonary, geriatric, neurological, orthopedic, and oncology cases
• Expected caseload of 6–8 patient visits per day (minimum 6 points per day, may increase at manager's discretion)
• Day shift, 5x8-hour shifts (8:00 AM – 5:00 PM)
• Every 3rd weekend required
• Self-scheduled visits and hours based on the needs of the home care office
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet for documentation
• Laptop, MiFi device, and portable printer provided for field use
• Local candidates accepted

Requirements

• Minimum 2 years of physical therapy experience required
• Home health experience required
• Current BLS certification required
• Valid state PT license required if applicable
• Valid driver's license and personal vehicle required for home visits
• Proficiency with adaptive equipment including crutches, gait belts, walkers, sliding boards, collars, and supplemental oxygen delivery devices
• Skills in assessment, documentation, patient/family education, home exercise program development, and safety/injury prevention
• COVID vaccination or approved exemption form required
• Flu shot or signed declination required (mandatory masking at all times if declined)

Additional Information

• OASIS training is preferred
• Responsibilities include skilled home assessments, therapeutic interventions, patient and family education, and documentation across a diverse caseload
• Must be able to recognize signs and symptoms of physical, sexual, and psychological abuse
• Any solid color scrubs accepted
• 2 professional references from a supervisor within the past 2 years required for consideration
• Mandatory masking at all times while on the job for candidates who decline the flu vaccine
