Description
American Traveler is seeking an experienced RN Case Manager for a contract position at a rehabilitation hospital in Cambridge, MA, requiring an active MA RN license and Epic experience.
Details
- Day shift position, 8:00 AM – 4:30 PM
- Case Management unit within a rehabilitation hospital setting in Cambridge, MA
- Responsibilities span both inpatient coordination and referrals for post-discharge community services
- Uses Epic for charting
- UKG/Kronos used for timekeeping
Requirements
- Active MA RN license required — no pending or conditional licenses accepted
- Minimum 2 years of case management experience required
- BLS certification required — AHA only; Red Cross and other providers are not accepted for this facility
- Epic EMR experience required
- 2 professional references required for consideration
- Flu vaccine required; no religious or medical exemptions are accepted
Additional Information
- Conducts client assessments and develops individualized care plans in collaboration with patients, families, and service providers
- Modifies treatment plans based on patient responses and changing conditions
- Communicates with third-party payers to obtain authorizations for reimbursement of services
- Coordinates discharge planning and prepares patients and families for transitions to community-based care
- Maintains accurate documentation and detailed case records
- Collaborates with the medical team on complex or challenging cases
- Local candidates with a permanent residence within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
- No validated parking is available at Boston-area locations; public transportation is recommended, and public parking rates range from $17–$22 per day
- Any requested time off (RTO) must be disclosed at the time of consideration
