Description

American Traveler is seeking an experienced RN Case Manager for a contract position at a rehabilitation hospital in Cambridge, MA, requiring an active MA RN license and Epic experience.

• Day shift position, 8:00 AM – 4:30 PM
• Case Management unit within a rehabilitation hospital setting in Cambridge, MA
• Responsibilities span both inpatient coordination and referrals for post-discharge community services
• Uses Epic for charting
• UKG/Kronos used for timekeeping

Requirements

• Active MA RN license required — no pending or conditional licenses accepted
• Minimum 2 years of case management experience required
• BLS certification required — AHA only; Red Cross and other providers are not accepted for this facility
• Epic EMR experience required
• 2 professional references required for consideration
• Flu vaccine required; no religious or medical exemptions are accepted

Additional Information

• Conducts client assessments and develops individualized care plans in collaboration with patients, families, and service providers
• Modifies treatment plans based on patient responses and changing conditions
• Communicates with third-party payers to obtain authorizations for reimbursement of services
• Coordinates discharge planning and prepares patients and families for transitions to community-based care
• Maintains accurate documentation and detailed case records
• Collaborates with the medical team on complex or challenging cases
• Local candidates with a permanent residence within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
• No validated parking is available at Boston-area locations; public transportation is recommended, and public parking rates range from $17–$22 per day
