Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week contract covering East Portland, OR and surrounding areas.
Details
- Home health setting serving East Portland, OR and surrounding areas
- 8-hour day shifts, Monday–Friday; 4x10 and 3x12 schedules cannot be accommodated
- Caseload of up to 6 patients per day with a standard productivity expectation of 30 points
- 1:1 patient ratio
- Travel required for patient visits; the system groups clients to minimize drive time, with an occasional maximum daily radius of roughly 60 miles (60–90 minute total drive time)
- Charting via Epic Remote Client for Home Health (laptop) and Rover app (phone)/OASIS
- No on-call or weekend requirements
- No floating between like units required
Requirements
- Minimum 1 year of skilled home health PT experience required
- Current BLS certification required
- Epic EMR experience required
- OASIS experience required
- Active PT license required if applicable
- Must have reliable personal transportation
- Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting
- SSN and DOB required for consideration
Additional Information
- First-time travelers are welcome to apply
- Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate
- Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility
- All travelers are expected to float between facilities within the state; refusal to float may result in assignment termination
- Minimal RTO is preferred; any RTO or scheduling requests must be disclosed prior to submission, as post-offer requests may be denied
- Business casual attire required; scrubs are permitted in any color but not required — no jeans
- Mileage reimbursed at the IRS standard rate; parking is free
