Description

American Traveler is seeking a Physical Therapist - Home Health Case Manager with at least 1 year of recent home health experience to provide in-home patient care across a defined coverage area in the Chicago metro region.

Details

• 100% field-based, remote position within the Central Metro E coverage area (ZIP codes: 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas)
• Patient population consists of adult and geriatric general med/surg patients
• Current team census is approximately 200 patients
• Expected caseload of 6 patient visits per day within the assigned coverage area
• EMR: Epic (experience required)
• OASIS documentation experience required
• Schedule is Monday–Friday, 8am–5pm
• No weekend rotation, no on-call requirements, and no holiday coverage expectations
• Supplies and equipment are delivered directly to the traveler's home

Requirements

• Minimum 1 year of recent home health experience with adult/geriatric patients required; 2 years preferred
• Active IL PT license required (license required if applicable)
• Current BLS certification (AHA) required
• Master's degree in Physical Therapy required
• Experience with Epic EMR required
• Experience with OASIS documentation required
• Proficiency in assisting patients with ADLs and adaptive equipment required
• Ability to effectively collaborate with interdisciplinary team members (RN, OT, Speech, etc.)
• Valid driver's license and active auto insurance required; candidate must provide their own reliable transportation to all patient visits
• Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN, dated within the past year
• Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

Additional Information

• Approximately 2 days of on-site orientation held in Oak Brook, IL, covering EMR training and skills check-off; all subsequent work and meetings are conducted remotely or in the field
• Supervised by a nursing supervisor, manager, and team leads
• Floating occurs within the designated Central Metro E coverage area only
• Any solid color scrubs acceptable; business casual attire also permitted — no jeans or open-toe shoes
• Local candidates are accepted with no radius restrictions
• First-time travelers may be considered for this position
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
