Description

American Traveler is hiring an experienced RN for a Hospice/Palliative Care Case Manager position requiring HCHB EMR experience and a minimum of one year of recent hospice experience.

Details

Home hospice setting serving a territory of up to 60 miles from the hub/office
Geriatric end-of-life care patient population
Average caseload of 5–7 patients per day, with a minimum of 30 points per week
Monday through Friday, 8:00 AM – 5:00 PM (5x8-hour days)
Back-up on-call responsibilities required; details to be discussed during interview
HCHB (HomeCarHomeBase) EMR experience is required
Local candidates are accepted at the travel rate

Requirements

Active RN license (Standard or Compact) currently in hand
Current BLS certification required
Minimum 1 year of recent hospice nursing experience required
HCHB EMR experience required
Must be flexible with territory coverage within the assigned service area

Additional Information

Provide case management and direct nursing care to hospice patients in a home-based setting
Timely documentation and communication are essential responsibilities of this role
iPad device provided; personal cell phone is not required
Mileage reimbursed at the IRS standard rate
Scrubs required in any color; jeans are not permitted
First-time travelers will not be considered for this position
Any time off requests must be submitted at the time of initial consideration
Full 7-year work history with all gaps explained is required, along with 2 professional supervisory references from within the past 2 years
