Description
American Traveler is hiring an experienced RN for a Hospice/Palliative Care Case Manager position requiring HCHB EMR experience and a minimum of one year of recent hospice experience.
Details
- Home hospice setting serving a territory of up to 60 miles from the hub/office
- Geriatric end-of-life care patient population
- Average caseload of 5–7 patients per day, with a minimum of 30 points per week
- Monday through Friday, 8:00 AM – 5:00 PM (5x8-hour days)
- Back-up on-call responsibilities required; details to be discussed during interview
- HCHB (HomeCarHomeBase) EMR experience is required
- Local candidates are accepted at the travel rate
Requirements
- Active RN license (Standard or Compact) currently in hand
- Current BLS certification required
- Minimum 1 year of recent hospice nursing experience required
- HCHB EMR experience required
- Must be flexible with territory coverage within the assigned service area
Additional Information
- Provide case management and direct nursing care to hospice patients in a home-based setting
- Timely documentation and communication are essential responsibilities of this role
- iPad device provided; personal cell phone is not required
- Mileage reimbursed at the IRS standard rate
- Scrubs required in any color; jeans are not permitted
- First-time travelers will not be considered for this position
- Any time off requests must be submitted at the time of initial consideration
- Full 7-year work history with all gaps explained is required, along with 2 professional supervisory references from within the past 2 years
