Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week travel assignment in the Seaside, OR area.
Details
- Home health setting serving Seaside, OR and surrounding areas
- 8-hour day shifts, 5 days per week (4x10 and 3x12 schedules are not available)
- Up to 6 patient visits per day with a standard productivity expectation of 30 points
- 1:1 patient ratio
- Charting via Epic Remote Client (laptop) and Rover (phone app); OASIS documentation required
- No weekend or on-call requirements
- Travelers are expected to float between all facilities within the state; refusal to float may result in assignment termination
Requirements
- Minimum 1 year of skilled home health PT experience required
- Current BLS certification required
- Epic EMR experience required
- OASIS experience required
- Active PT license required; pending license accepted
- Must have reliable personal transportation
- Primary COVID-19 vaccine series plus one booster required
- Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
- SSN and DOB required for consideration
- License required if applicable
Additional Information
- Responsibilities include skilled home health physical therapy evaluation and treatment across an assigned geographic territory
- Scheduling system groups patients to minimize drive and windshield time; occasional daily driving radius of up to approximately 60 miles (roughly 60–90 minutes total drive time)
- Business casual attire required; scrubs are acceptable but no jeans and no specific color requirement
- Local candidates within a 50–75 mile radius are accepted at the same rate
- Former permanent staff must wait at least 3 months before returning as a traveler and cannot be located within 50 miles of the submitting facility
- Open to first-time travelers
- RTO requests should be confirmed with the hiring manager prior to submission; minimal RTO is preferred
