Description
American Traveler is hiring a travel RN for a Home Hospice position on the Supportive Care Team (SCT) serving the Green Bay and Oshkosh markets, requiring a minimum of 2 years of home health or home hospice experience and an active WI or compact RN license.
Details
- Home hospice setting — RN will travel to patients' homes across assigned territory in the Green Bay and Oshkosh markets
- Territory coverage includes communities across Green Bay (e.g., De Pere, Pulaski, Kewaunee, Algoma) and Oshkosh (e.g., Appleton, Neenah, Oshkosh, Winneconne) regions
- Patient population includes those with failure to thrive, advanced cancers, end-stage COPD, end-stage disease processes, and terminal illness
- Visits include after-hours PRN visits for symptom management, falls, and death visits; some issues may be resolved by phone
- Patient caseload ranges from 0–6 patients per shift, depending on call-ins
- 12-hour day shifts, 8:00 AM – 8:00 PM, Friday, Saturday, and Sunday
- 36 hours per week on a weekend-only schedule
- Every-other-weekend rotation
- No on-call requirements
- EMR: Epic (Rover); Epic experience preferred
- Equipment includes Sapphire IV pump (rarely encountered in the home setting)
Requirements
- Active WI RN license or compact RN license currently in hand
- Current BLS certification required
- Minimum 2 years of home health or home hospice experience required
- Valid driver's license, personal vehicle, and active auto insurance required
- Comfortable performing focused assessments, critical thinking, and clinical problem-solving in an independent home setting
- Skills required include foley catheter exchange, feeding tube troubleshooting, wound care, post-fall assessment, narcotic medication administration, and identification of potential drug diversion
- Comfortable conducting death visits and communicating with medical examiners or coroners
- Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the past year required for consideration
- Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration
Additional Information
- Responsibilities include conducting in-home patient assessments, managing symptoms, responding to after-hours triage calls, and providing end-of-life care
- Lead RN is available 24/7 for support; RNs are expected to update the Lead RN after visits and provide report
- Other SCT members are accessible by phone for additional support
- Orientation includes new employee onboarding at the territory office followed by 3 shifts (approximately 1 week) with an experienced SCT nurse
- May occasionally float slightly outside assigned territory when closer to an incoming call; never more than one hour from home territory
- Required to work one summer and one winter holiday; Fourth of July coverage is specifically required
- RTO requests must be approved by the manager

- First-time travelers with case management experience in the home setting will be considered
- Local candidates are accepted
- Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
