Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience for a 13-week travel assignment on the beautiful island of Kaua'i, Hawai'i.

Details

• Home health setting serving a primarily geriatric patient population (kupuna) in their homes on Kaua'i
• Patient cases include orthopedic (total hip/knee replacement, post-op spinal surgery), neurological (stroke, Parkinson's, dementia), and cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD)
• Clinical responsibilities include evaluation, OASIS assessments, plan of care development, short/long-term goal setting, reassessments, and discharge planning
• Productivity requirement of 25 units per week
• Documents using MatrixCare
• Day shift schedule: 5x8-hour shifts, 8:00 AM – 4:30 PM
• Occasional floating to cover Oahu, Maui, and/or the Big Island may be required; all travel expenses covered

Requirements

• Minimum 1 year of recent home health PT experience required
• Current HI PT license required (license required if applicable)
• BLS certification (AHA) required
• Proficiency in geriatric, neurological, and orthopedic patient care required
• Experience with OASIS documentation and home health plan of care processes required
• Skills in gait training, adaptive/assistive device use, home exercise program development, and patient/family education required
• Ability to identify environmental barriers and perform home safety assessments required
• Closed-toed shoes required

• Scrubs in any color are acceptable
• First-time travelers are welcome to apply
• Preferred skills include wound care, lymphedema management, acute care experience, and experience with MS, muscular dystrophy, spinal cord injury, TBI, and NDT techniques
