Description
American Traveler is hiring an experienced RN for a Hospice Case Manager position in a home health and hospice setting, requiring prior hospice case management experience and a VT or Compact RN license.
Details
- Home health and hospice agency setting serving a rural patient population
- Hospice case management role with in-home patient care
- Monday through Friday schedule
- 20-week contract
Requirements
- Active VT or Compact RN license currently in hand
- Current BLS/BCLS certification required
- Minimum 1 year of RN experience required
- Prior hospice case management experience required
- Experience providing hospice care in a home setting required
- Prior experience with rural travel preferred
- McKesson/Netsmart EMR experience preferred
Additional Information
- Candidate should be available for the full duration of the contract with no time-off requests preferred
- SSN and DOB will be required for consideration
