Description

American Traveler is hiring a Home Health Occupational Therapist with a minimum of 1 year of home care experience for a position serving the Norwalk and surrounding areas of Connecticut.

Details

• Home health setting serving patients in the South territory, including Norwalk, Darien, New Canaan, Stamford, Westport, Wilton, and Weston; may occasionally cover the Danbury area
• Day shift schedule, 8:30 AM – 5:00 PM
• Diverse patient caseload including post-orthopedic surgery patients and heart failure and COPD patients requiring energy conservation therapy
• OT serves as the primary therapy discipline for heart failure and COPD patients
• Home Care Home Base (HCHB) EMR experience preferred

Requirements

• Current, active OT license in the state of Connecticut (license required if applicable)
• Minimum 1 year of home care experience
• Current AHA BLS certification required
• Two professional references rated 'standard or above average' from a manager or above within the prior year in the current discipline/specialty

Additional Information

• Provide direct OT services focused on upper extremity function, ADL independence, and cognitive-perceptual-motor function
• Deliver energy conservation training for heart failure and COPD patients
• Maintain accurate visit records including evaluations, care plans, and client progress notes
• Educate patients and family members on OT treatment regimens
• Supervise COTAs and home health aides as appropriate
• Candidates living within 60 miles of the facility are eligible to apply at a reduced rate
• Candidates who have previously worked at this health system may be subject to an eligibility review process
