Description

American Traveler is seeking a licensed Physical Therapist with 2+ years of experience for a 13-week home health assignment in Yolo County, CA, utilizing Homecare Homebase (HCHB) for documentation.

Details

• Home health care setting serving patients across Yolo County, including Woodland, Davis, West Sacramento, Vacaville, Winters, and surrounding communities
• Standard schedule is days, 8:30a–5:00p with 8-hour shifts
• Rotating Saturday coverage approximately once per month
• Uses Homecare Homebase (HCHB) as the EMR
• Post-acute care setting

Requirements

• Minimum 2 years of PT experience in an acute, home care, or rehab setting
• Active state PT license required (pending licenses not accepted); license required if applicable
• Active BLS certification required (pending certifications not accepted)
• Strong oral and written communication and organizational skills
• Driver's license required for consideration
• 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire

Additional Information

• Responsibilities include assessing, evaluating, and treating patients in the home setting per physician plan of care, including muscle strength, mobility, gait, and ROM evaluations
• Duties include instructing patients, families, and caregivers in therapeutic exercises, use of appliances, and rehabilitation goals
• May supervise home health aides participating in a patient's rehabilitation regimen
• Prepares and submits clinical and progress summaries and reports changes in patient condition to the attending physician
• Mileage is reimbursable per GSA guidelines beginning after departure from the first patient visit of the day
• Scrubs are not required; acceptable attire includes any color scrubs or slacks (khakis acceptable) with a polo or button-up shirt
• Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity-affiliated facility within the past year are not eligible; candidates employed PRN through outside staffing firms will be considered
