Description
American Traveler is seeking an experienced RN Case Manager for a community hospice travel position requiring a minimum of 2 years of experience and an active CO or compact RN license.
Details
- Community hospice setting, providing care wherever patients call home — including private residences, nursing homes, group homes, and assisted living facilities
- Assigned caseload of patients as the primary RN, leading an interdisciplinary care team
- Monday–Friday, 8:00 AM–5:00 PM schedule
- Occasional weekend backup and on-call responsibilities
- 12-week contract
Requirements
- Active CO or compact RN license currently in hand
- Minimum 2 years of RN experience required
- Strong clinical assessment and critical thinking skills
- Experience in hospice, palliative care, or case management preferred
Additional Information
- Primary responsibilities include symptom management, patient goal-of-care planning, and educating and supporting family caregivers
- Functions as a generalist across disease types with a specialty focus on end-of-life symptom management
- Serves as the lead RN within an interdisciplinary care team for an assigned patient caseload
- DOB and SSN may be required for consideration
