Description

American Traveler is seeking an experienced RN for a Home Health position in the Greensboro/High Point, NC area, requiring prior case management experience and HCHB proficiency.

Details

  • Community-based home health setting — not an inpatient position
  • Covers the Greensboro and High Point service areas
  • Role involves case managing an active patient caseload
  • Monday–Friday, day shift schedule (8:00 AM – 5:00 PM)
  • 13-week contract

Requirements

  • Active NC RN license or compact RN license currently in hand
  • Minimum 1 year of case management experience required
  • Homecare Homebase (HCHB) EMR experience required
  • Prior home health travel experience required
  • Must have a personal vehicle for daily travel throughout the service area
  • Two professional references required for consideration
  • PDF copies of driver's license and Social Security card required for consideration

Additional Information

  • Responsible for managing and coordinating care for an assigned home health patient caseload
  • Will travel independently between patient homes throughout the Greensboro and High Point areas throughout the workday
  • Mileage reimbursement provided at the federal rate
