Description
American Traveler is seeking a Physical Therapist with home health experience for a 13-week travel contract serving patients across the San Francisco Bay Area.
Details
- Home health setting serving patients across San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo, and neighboring cities
- Day shift, 8:00am–5:30pm, with schedule flexibility based on patient availability
- 13-week contract
Requirements
- Minimum 1 year of home health PT experience required
- Current and active state PT license required (license required if applicable)
- Current BLS/BCLS certification required
Additional Information
- Responsibilities include evaluating and treating patients per medically prescribed physical therapy programs, with a focus on restoring function and maximizing patient performance
- May supervise Physical Therapy Assistants as part of care team responsibilities
- Conducts initial admission and evaluation visits, including OASIS scoring documentation within established timelines
- Documents clinical findings, skilled interventions, goals, and discharge plans for each patient
