Description

American Traveler is seeking an experienced RN for a Clinical Care RN role in a PACE (Program of All-inclusive Care for the Elderly) setting, requiring at least one year of experience with frail or elderly populations and a valid MI RN license.

Details

Day shift schedule: Monday–Friday, 8:00 AM–4:30 PM
Work setting includes both a clinic/care center environment and patient home visits
Patient population consists of frail, elderly, and long-term care participants with acute and chronic medical conditions
13-week contract
Home visits are conducted on a regular basis as ordered by the Primary Care Provider (PCP)
Services provided include wound care, lab draws, nurse assessments, and other nursing tasks
Collaborates with licensed and non-licensed personnel, as well as PCPs

Requirements

Active MI RN license currently in hand
Current BLS/BCLS certification (AHA)
Associate's degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred
Minimum 1 year of experience working with frail, elderly, or long-term care populations
Home care experience preferred (minimum 1 year)
Wound care experience preferred (minimum 1 year)
Must have reliable transportation, a valid Michigan driver's license, and current auto insurance
Must be 18 years of age or older
Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
DOB and SSN required for consideration

Additional Information

Triage emergent situations and apply advanced nursing knowledge independently to manage complex clinical cases
Monitor acute and chronic medical conditions in coordination with PCPs and utilize care plans to provide clinic- and home-based direct care
Administer treatments, procedures, and medications as prescribed
May be required to work beyond normal hours, including weekends and holidays as needed
Role operates with a high degree of independence in clinical decision-making
Frequent interaction with staff, participants, caregivers, and external organizations
