Description
American Traveler is seeking an experienced RN for a Clinical Care RN role in a PACE (Program of All-inclusive Care for the Elderly) setting, requiring at least one year of experience with frail or elderly populations and a valid MI RN license.
Details
- Day shift schedule: Monday–Friday, 8:00 AM–4:30 PM
- Work setting includes both a clinic/care center environment and patient home visits
- Patient population consists of frail, elderly, and long-term care participants with acute and chronic medical conditions
- 13-week contract
- Home visits are conducted on a regular basis as ordered by the Primary Care Provider (PCP)
- Services provided include wound care, lab draws, nurse assessments, and other nursing tasks
- Collaborates with licensed and non-licensed personnel, as well as PCPs
Requirements
- Active MI RN license currently in hand
- Current BLS/BCLS certification (AHA)
- Associate's degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred
- Minimum 1 year of experience working with frail, elderly, or long-term care populations
- Home care experience preferred (minimum 1 year)
- Wound care experience preferred (minimum 1 year)
- Must have reliable transportation, a valid Michigan driver's license, and current auto insurance
- Must be 18 years of age or older
- Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
- DOB and SSN required for consideration
Additional Information
- Triage emergent situations and apply advanced nursing knowledge independently to manage complex clinical cases
- Monitor acute and chronic medical conditions in coordination with PCPs and utilize care plans to provide clinic- and home-based direct care
- Administer treatments, procedures, and medications as prescribed
- May be required to work beyond normal hours, including weekends and holidays as needed
- Role operates with a high degree of independence in clinical decision-making
- Frequent interaction with staff, participants, caregivers, and external organizations
