Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week travel assignment in the Medford, OR area.
Details
- Home health setting serving Medford, OR and surrounding areas
- 5x8-hour day shifts (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available
- Caseload of up to 6 patients per day with a standard productivity expectation of 30 points
- 1:1 patient ratio
- Charting is done in Epic and OASIS
- No weekend or on-call requirements
- Travelers are expected to float between all facilities within the state
- Occasional patient visits may require driving up to 50+ miles, though the system is designed to minimize drive time and windshield time
- Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate
Requirements
- Minimum 1 year of skilled home health PT experience required
- Current BLS certification required
- Epic EMR experience required
- OASIS experience required
- Active PT license required (pending license accepted); license required if applicable
- Must have reliable personal transportation
- Primary COVID-19 vaccination series plus one booster required
- Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting
- SSN and DOB required for consideration
Additional Information
- First-time travelers will not be considered for this position
- Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility
- Any RTO requests should be disclosed upfront, as post-offer RTO requests may be denied
- Any color scrubs are acceptable
- Parking is free at facility locations
- Mileage reimbursement is provided at the IRS standard rate
