Description
American Traveler is seeking an SLP for a home health position in Morristown, NJ requiring a Master's degree, active NJ license, and at least 1 year of experience in an acute, subacute, or home care setting.
Details
- Home health setting with travel to patient homes and facilities
- Monday through Friday, 8am to 4pm; no weekends
- 40 hours per week, 8 hours per day
- May be required to float to a nearby sister facility as needed
- Uses Epic for charting
- No on-call required
Requirements
- Master's degree from an accredited speech-language pathology program
- Active and unrestricted NJ Speech-Language Pathology license (license required if applicable)
- Minimum of 1 year of experience in an acute, subacute, or home care setting
- Home health experience required
- Strong customer service and communication skills
- Use of a personal vehicle is required for home visits
- 2 professional references required for consideration
Additional Information
- Perform baseline assessments of patient swallowing, communication, and cognitive abilities
- Collaborate with patients, families, and physicians to develop and revise individualized plans of care
- Administer speech therapy services per physician orders, including communication, safe swallow, and cognitive training
- Assess social environment and clinical risks, adjusting programs and providing education as needed
- Provide discharge planning and referral to appropriate resources
- Communicate and coordinate with physicians, nurses, and other clinical disciplines through IDT meetings
- Document patient education, plan of care, and progress in accordance with agency policies and home care regulations
- Demonstrate knowledge of home care regulations and insurance reimbursement guidelines
- Manage caseload and prioritize scheduling to meet patient needs and productivity standards
- Candidates must reside more than 50 miles from the facility to be eligible for this position
