Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of experience and Homecare Homebase EMR proficiency.

Details

• Home health setting covering York and Lancaster counties, SC, with occasional float coverage to Union or Mecklenburg County
• Adult patient population with chronic disease management, wound care, IV therapy, pleurx, and wound vac cases
• Caseload of approximately 6 visits or points per day
• 8-hour day shift schedule, 0800-1700, 40 hours per week
• No on-call or weekend rotation requirements
• Holiday coverage required as scheduled
• Uses Samsung Note 9 phones and Homecare Homebase EMR for documentation

Requirements

• Minimum 1 year of recent experience in home health or related specialty
• BLS certification required
• State PT licensure required if applicable
• BA degree required as minimum education level
• Homecare Homebase EMR experience required, no exceptions
• OASIS-E assessment tool experience required
• Experience with IV therapy, pleurx, wound vac, wound care, and disease teaching required
• Experience with geriatric and adult chronic disease management and maintenance notes required
• Valid driver's license and proof of insurance required
• Last 4 digits of SSN and full date of birth required for consideration
• One professional reference dated within the past year from a Supervisor, Manager, or Director required

Additional Information

• First-time travelers considered if experienced in home health
• Five-day orientation through New Employee Orientation (NEO)
• Grey scrub color required for therapy staff
• Radius rule requires permanent residence at least 50 miles outside the home unit; local candidates not accepted
• Weekend-only scheduling may require travel up to 100 miles depending on caseload
• Clinical interview required with final approval from hiring manager
• Returning employees or travelers must be separated from any AHSE facility for at least one year after working a permanent position
