Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of experience and Homecare Homebase EMR proficiency.

Details

• Home Health setting covering York and Lancaster counties in South Carolina
• Adult patient population only
• Common diagnoses include chronic diseases, wound care, IV therapy, pleurx, and wound vac management
• Approximately 6 total visit points per day
• 8-hour day shift schedule, 0800-1700, 40 hours per week
• Weekend-only positions may require driving up to 100 miles depending on caseload
• May occasionally float into Union or Mecklenburg County, though not likely
• Uses Homecare Homebase EMR system
• Utilizes OASIS-E assessment tool
• Equipment includes Samsung Note 9 phones
• No on-call or call-back requirements
• No weekend rotation required
• Will work holidays as scheduled

Requirements

• Minimum 1 year of core work experience required
• BLS certification required
• State licensure required (license required if applicable)
• BA degree required as minimum education level
• Homecare Homebase EMR experience required with no exceptions
• OASIS-E assessment tool experience required
• Experience with IV therapy, pleurx, wound vac, and wound care required
• Geriatric and adult chronic disease management experience required
• Experience with maintenance notes and disease teaching required
• Valid driver's license and proof of insurance required
• Last 4 digits of SSN and full date of birth required for consideration

Additional Information

• First-time travelers considered if seasoned in home health
• Clinical interviewing required, with final approval from manager
• Departmental leadership provides support
• 5-day facility and unit orientation included as part of onboarding
• Grey scrub color designated for therapy staff
• Traveler's permanent address must be more than 50 miles from the home unit
• Returning travelers must be separated from any AHSE facility for at least 1 year following a permanent position
