Description
American Traveler is seeking a licensed Physical Therapist with at least 1 year of experience for a 13-week home health contract in the Louisville Metro area, utilizing Homecare Homebase (HCHB) for documentation.
Details
- Home health care setting serving the Louisville Metro area
- Common patient diagnoses include CHF, orthopedic conditions, and pneumonia
- Expected caseload of 6.5 points per day, including start of care (SOC), resumption of care (ROC), recertifications, evaluations, and routine visits
- Monday–Friday, 8:00 AM–5:00 PM with rotating weekend shifts (8:00 AM–5:00 PM)
- 8-hour shifts, 5 days per week
- Charting performed in Homecare Homebase (HCHB)
- Holiday rotation is required
Requirements
- Minimum 1 year of physical therapy experience; new graduates are less preferred
- Active state or compact (NLC) PT license required at time of consideration — license required if applicable; pending licenses will not be accepted
- Active BLS certification required; pending certifications will not be accepted
- Driver's license required for consideration
- 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire
Additional Information
- Responsibilities include conducting SOC, ROC, recertification, evaluations, and ongoing patient visits in a home health setting
- A clinical manager or educator is available 24/7 for emergencies and 8:00 AM–5:00 PM for general support while in the field
- Orientation consists of approximately 2 weeks with a work coach, including supervised patient visits; no formal orientation hours are provided
- Phone and tablet will be provided for use in the field
- Scrubs or business casual attire are both acceptable
- Local candidates are accepted
