Description
American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring a minimum of 1 year of home health experience and a DPT.
Details
- Home Health setting covering 13 counties, with team rotation weekly through the furthest service areas
- Counties served include Andrew, Atchison, Buchanan, Clay, Clinton, Daviess, Dekalb, Gentry, Harrison, Holt, Platte, and Worth
- Fewer patients scheduled on days with high mileage or long travel times
- Shift options include 5x8 Days (0800-1630) or 4x10s if preferred
- Call required approximately 1 Sunday every 5 weeks
- Every 5th week includes working Saturday and Sunday for admits, with 2 weekdays off in exchange
- Uses Epic for charting
Requirements
- Minimum 1 year of experience required, with home health experience specifically required
- Current BLS certification required
- Doctor of Physical Therapy (DPT) required
- State license required if applicable
- Epic charting experience preferred
- Previous travel experience preferred
- Skilled in patient assessment, including AROM/AAROM/PROM, strength testing, vital sign monitoring, and skin integrity checks
- Experience with adaptive and assistive devices, including Hoyer lifts, walkers, and wheelchairs
- Knowledge of OASIS documentation, care planning, and RUG levels
- Experience treating cardiopulmonary, neurologic, and orthopedic conditions preferred
- Familiarity with home exercise programs and patient/family education
Additional Information
- Responsibilities include conducting evaluations, developing plans of care, and performing start-of-care (SOC) assessments independently
- Provides skilled therapy services in patients' homes across a wide geographic area
- Scrub requirements: no print, black, grey, or khaki pants with polo or scrub top
- Local candidates accepted at the same rate
- Traveler's permanent address must be at least 50 miles from the facility for travel rate eligibility
- Previous employees must be separated from the facility for at least 1 year before returning as a traveler
- Orientation includes required training modules completed through MyPath
- RTO must be submitted at time of consideration
- Pending state licenses will be considered, provided the license is active and in-hand prior to the start date
