Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract providing PT services to patients in their place of residence across the North Shore region of Massachusetts.

Details

• Home health setting serving patients in their place of residence across multiple communities in the North Shore region of Massachusetts, including Boxford, Danvers, Hamilton, Lynnfield, Middleton, North Andover, North Reading, Peabody, Topsfield, and Wenham
• Float role requiring travel to patient cases as assigned throughout the service territory
• Day shift schedule: 8:00 AM – 4:30 PM
• Weekend rotation required once every 6 weeks
• Patient population ranges from newborns to geriatric, with conditions including physical impairments, functional limitations, and disabilities resulting from injury, disease, or other causes
• EMR: Epic (charting system used for documentation)

Requirements

• Active Massachusetts PT license required; MA conditional licenses are not accepted
• Minimum 1 year of experience in a certified home health agency required; experience in hospice, private care, or case management alone does not satisfy this requirement
• Minimum 1 year of case management experience required
• AHA certifications only accepted
• Flu vaccine required; no religious or medical exemptions are accepted
• Must have reliable transportation to travel to patient cases throughout the assigned service area
• License required if applicable

Additional Information

• Primary responsibilities include evaluating and treating patients with physical impairments, functional limitations, and disabilities in their home environment
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Timely and thorough electronic documentation and interdisciplinary communication required in accordance with agency policies and standards of practice
• 2 professional references required for consideration
• Any requested time off (RTO) should be disclosed at the time of application
• Local candidates residing within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
• Parking is not validated at Boston-area locations; public parking is available at candidate expense, with daily rates typically ranging from $17–$22; other locations may also have associated parking costs
• Timekeeping is managed through UKG/Kronos
