Description

American Traveler is seeking a compassionate RN for a Hospice Care Manager position in a home health setting in Vermont, requiring OASIS experience and an active VT or compact RN license.

Details

Home health and hospice setting, providing care in patients' homes
Hospice and care management patient population, focused on end-of-life care
Day shift schedule, Monday through Friday with some weekend coverage
16-week contract

Requirements

Active VT or compact RN license currently in hand
Current BLS/BCLS certification required
Minimum 1 year of RN experience required
Home health experience required
OASIS documentation experience required
Case management experience required

Additional Information

Provide compassionate end-of-life care, addressing the physical and emotional needs of patients and their families
Deliver expert pain and symptom management to hospice patients
Collaborate with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life
DOB and SSN may be required for consideration
