Description

American Traveler is seeking an experienced RN for a Home Health position in Vermont, serving as both a direct care nurse and Case Manager on a Monday–Friday schedule with a weekend rotation.

Details

Home health setting serving a community-focused patient population in central Vermont
Schedule is Monday–Friday with a weekend rotation approximately every 4th week
Day shift position
Serves as Case Manager when acting as the lead discipline on a case

Requirements

Active VT or Compact RN license currently in hand
Current BLS/BCLS certification required
Minimum 1 year of RN experience required
DOB and SSN required for consideration

Additional Information

Responsible for providing individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
Provides a range of services including home health, health promotion, long-term care, and maternal-and-child health services
