Description

American Traveler is hiring an RN Case Manager for a Hospice/Palliative Care position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a valid VA or compact RN license.

Details

Community/home-based hospice setting operating out of a hub office in Loudoun County, VA
Geriatric patient population requiring end-of-life care
Caseload of approximately 5–7 patients per day
Day shift schedule, 5x8-hour shifts (8:00 AM – 5:00 PM)
No on-call responsibilities
Territory covers up to a 60-mile radius from the hub office; flexibility with territory is expected
EMR: Netsmart
iPad provided for use in the field

Requirements

Active VA RN license or compact RN license currently in hand
Current BLS certification required
Minimum 1 year of recent hospice nursing experience required
Strong time management skills with an emphasis on timely documentation and communication

Additional Information

Responsibilities include managing a caseload of hospice patients in a community/home-based setting across an assigned territory
Scrubs required in any color; jeans are not permitted
Mileage is reimbursed at the IRS standard rate
Local candidates are welcome to apply
Two professional references from a supervisor within the past 2 years are required for consideration
All time-off requests must be disclosed upfront during the consideration process

My phone showed her number as spam and I think blocked some of the texts I wish it hadn't have done that but that was nothing on her end all on mine so make sure that you have your settings to allow the calls and texts from spam
