Description

American Traveler is seeking an experienced RN for a Clinical Care (PACE) position in Battle Creek/Albion, MI, requiring at least 1 year of experience with frail or elderly populations and a valid MI RN license.

Details

PACE (Program of All-Inclusive Care for the Elderly) clinic and community-based setting
Geriatric and frail elderly patient population with acute and chronic medical conditions
Regular home visits required to provide care, wound care, lab draws, and nursing assessments as ordered by the PCP
Day shift, Monday through Friday, 8:00 AM – 4:30 PM
Occasional weekend and holiday coverage may be required
13-week contract

Requirements

Active MI RN license currently in hand
Current BLS/BCLS certification (AHA)
Minimum Associate's degree in Nursing from an accredited program; BSN preferred
Minimum 1 year of experience working with frail, elderly, or long-term care populations
Home care experience preferred
Wound care experience preferred
Valid Michigan driver's license, reliable transportation, and current auto insurance required
Must be 18 years of age or older
Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B

Additional Information

Provides direct nursing care to PACE participants both in the clinic and in their homes
Triages emergent situations and independently manages complex clinical cases using advanced nursing knowledge
Monitors and coordinates care for acute and chronic conditions in collaboration with Primary Care Providers (PCPs)
Administers treatments, procedures, and medications as prescribed
Collaborates with both licensed and non-licensed care team members
Works in a high-visibility center environment with frequent interruptions and ongoing communication with staff, participants, caregivers, and external organizations
