Description
American Traveler is seeking an experienced RN for a Clinical Care (PACE) position in Battle Creek/Albion, MI, requiring at least 1 year of experience with frail or elderly populations and a valid MI RN license.
Details
- PACE (Program of All-Inclusive Care for the Elderly) clinic and community-based setting
- Geriatric and frail elderly patient population with acute and chronic medical conditions
- Regular home visits required to provide care, wound care, lab draws, and nursing assessments as ordered by the PCP
- Day shift, Monday through Friday, 8:00 AM – 4:30 PM
- Occasional weekend and holiday coverage may be required
- 13-week contract
Requirements
- Active MI RN license currently in hand
- Current BLS/BCLS certification (AHA)
- Minimum Associate's degree in Nursing from an accredited program; BSN preferred
- Minimum 1 year of experience working with frail, elderly, or long-term care populations
- Home care experience preferred
- Wound care experience preferred
- Valid Michigan driver's license, reliable transportation, and current auto insurance required
- Must be 18 years of age or older
- Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
Additional Information
- Provides direct nursing care to PACE participants both in the clinic and in their homes
- Triages emergent situations and independently manages complex clinical cases using advanced nursing knowledge
- Monitors and coordinates care for acute and chronic conditions in collaboration with Primary Care Providers (PCPs)
- Administers treatments, procedures, and medications as prescribed
- Collaborates with both licensed and non-licensed care team members
- Works in a high-visibility center environment with frequent interruptions and ongoing communication with staff, participants, caregivers, and external organizations
