Description

American Traveler is seeking a Physical Therapist for a Home Health float position requiring an active Massachusetts PT license and 1 year of certified home care experience.

Details

• Home health setting providing physical therapy services to patients in their place of residence throughout the North Region, including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End
• Patient population ranges in age from birth to over 100 years old with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Shift schedule is 08:00-16:30
• Weekend rotations required once every 6 weeks
• Float position requiring ability to travel to cases as assigned
• Uses Epic for charting

Requirements

• Active Massachusetts Physical Therapist license required (license required if applicable)
• Minimum 2 years of physical therapy experience required
• Minimum 1 year of certified home health care experience required (hospice, private care, or standalone case management experience does not qualify)
• Minimum 1 year of case management experience required
• BLS certification required

Additional Information

• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate care
• Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice
• Flu vaccine required with no religious or medical exemptions allowed
• Local candidates accepted with a permanent residence within 60 miles of Boston, MA
• Start date is contingent on credentialing clearance; delays may require a rescheduled start date
