Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a float role covering the North Region of a major Boston-area home care network.

Details

• Home health setting serving patients in their place of residence across multiple Boston-area neighborhoods including Back Bay, Beacon Hill, Cambridge, Charlestown, South Boston, South End, and surrounding areas
• Float role covering the North Region scheduling unit
• Patient population ranges from newborns to elderly adults with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Day shift: 08:00–16:30
• Weekend rotation required once every 6 weeks
• Charting is done in Epic
• 13-week contract

Requirements

• Active Massachusetts PT license required; MA conditional licenses are not accepted — license required if applicable
• Minimum 2 years of physical therapy experience
• Minimum 1 year of experience in a certified home health agency required; experience in hospice, private duty, or standalone case management roles does not satisfy this requirement
• Minimum 1 year of case management experience required
• Ability to travel independently to patient cases as assigned
• All certifications must be issued by the American Heart Association (AHA)
• Flu vaccine required; no religious or medical exemptions are accepted
• 2 professional references required for consideration

Additional Information

• Responsibilities include evaluation and treatment of patients in their home, with a focus on restoring physical function and mobility
• Collaborates with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Timely and thorough electronic documentation is required in accordance with agency policies and MGH Home Care standards of practice
• Time and attendance tracked via UKG/Kronos
• No validated parking available at Boston locations; public parking costs approximately $17–$22 per day; public transportation is recommended
• Local candidates residing within 60 miles of the Boston facility are eligible but will receive a reduced pay rate
• Any requested time off must be disclosed at the time of consideration
