Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week float assignment serving the North Region (Middlesex area) of a major Massachusetts health system.

Details

• Home health setting — PT provides services directly in patients' places of residence
• Float position covering communities across the North Region, including Andover, Bedford, Billerica, Burlington, Concord, Lawrence, Lexington, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester, Woburn, and surrounding areas
• Patient population spans all ages, from birth to 100+, with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Shift hours: 08:00–16:30
• Weekend rotation required once every 6 weeks
• Documents using Epic EMR

Requirements

• Active Massachusetts PT license required; MA Conditional Licenses are not accepted
• License required if applicable
• Minimum 1 year of experience in a certified home health agency required; hospice, private care, or case management experience alone does not fulfill this requirement
• Minimum 1 year of case management experience required
• AHA-only certifications accepted
• Ability to travel independently to patient homes across the assigned region
• Flu vaccine required; no religious or medical exemptions are accepted
• 2 professional references required for consideration

Additional Information

• Responsibilities include evaluating and treating patients with physical impairments and functional limitations in their home environment
• Collaboration with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Timely and thorough electronic documentation and interdisciplinary communication required in accordance with agency policies and standards of practice
• Timekeeping completed through UKG/Kronos
• Local candidates residing within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
• RTO requests must be communicated at the time of consideration
• Parking is not validated at Boston-area locations; public transportation or paid public parking is available at candidate's expense
