Description
American Traveler is seeking an experienced RN Case Manager for an inpatient acute care hospital position requiring a CA RN license and a minimum of 5 years of RN Case Management experience.
Details
- Inpatient acute care hospital setting, Case Management unit
- Responsible for care coordination and care transitions across the acute care patient experience
- Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to manage patient progression and transitions
- May include assignment to the Emergency Department to address complex clinical and social situations to avoid unnecessary admissions
- Day shift, 5x8-hour shifts (8:00–16:30)
- Every other weekend required
Requirements
- Active CA RN license required
- Current BLS certification required
- Minimum 5 years of RN Case Management experience required
- Inpatient acute hospital Case Management experience is required
- 2 supervisor references obtained within the past year required for consideration
- Copies of all certifications required for consideration
Additional Information
- Conducts initial and ongoing patient assessments, including psychosocial and financial screenings, to develop comprehensive care coordination plans
- Performs utilization management including level of care reviews, length of stay monitoring, and collaboration with physicians on documentation and care decisions
- Manages care transitions by formulating discharge and post-acute plans in collaboration with patients, families, physicians, payers, and community resources
- Ensures compliance with CMS, Joint Commission, and other regulatory requirements related to transitions of care
- Identifies and refers quality, risk management, and psychosocial concerns to appropriate team members
- Candidates must reside more than 50 miles from the facility to qualify for travel pay rates
- Former staff must have been away from the facility for at least 1 year before returning as a traveler
- Only 1 holiday RTO request will be approved
