Description
American Traveler is seeking an experienced RN Case Manager for a 13-week acute inpatient care coordination position requiring an active MD or Compact RN license and at least 1 year of acute care nursing experience.
Details
- Acute inpatient hospital setting focused on care coordination and discharge planning
- Responsibilities span the acute inpatient population including medical/surgical, ED, and critical care patients
- Daytime shift: 8:30 AM – 5:00 PM, 8-hour shifts, 40 hours per week
- EMR: Epic (experience required)
Requirements
- Active MD or Compact RN license required — pending licensure not accepted
- All certifications must be active at time of consideration
- Associate's Degree in Nursing (ADN) required
- Minimum 1 year of experience in an acute medical/surgical, ED, or critical care nursing setting
- Experience with electronic medical record (EMR) systems required
- Knowledge of health care reimbursement systems including Medicare Advantage, Commercial payers, Medicaid, and CMS regulatory rules
- Last 4 digits of SSN and date of birth required for consideration
Additional Information
- Conducts initial face-to-face assessments for newly admitted patients within 24 hours to identify discharge needs
- Collaborates with physicians, nursing staff, utilization review, and the broader care team to develop and document appropriate transition plans
- Coordinates referrals to in-network post-acute vendors including Home Health, DME, IV Infusion, SNF Sub-Acute, and Acute Rehab
- Works with community providers and ambulatory case managers to facilitate safe discharges and prevent readmissions
- Local candidates are accepted — local and travel rates are the same
- Contingent workers may not exceed two years of total assignment time without a 90-day break, per facility policy
