Description
American Traveler is seeking a licensed Physical Therapist with 2+ years of experience for a 13-week Home Health assignment in the Sacramento, CA area, providing in-home and community-based care across a diverse patient population.
Details
- Care is delivered in patients' homes, assisted living facilities, senior apartments, and board and care homes
- Patient caseload is 3–5 patients per day, with up to 2 Start of Care visits per day or a combination of 3–4 evaluations
- Visit time is weighted to account for travel, documentation, and case management — e.g., a Start of Care visit is weighted at approximately 3.75–4 hours total
- Patient population is varied and includes orthopedic, cardiology, pulmonary, oncology, neurology, geriatric, and amputation cases; no pediatrics or hospice
- Service territory primarily covers the Sacramento area, with teams organized to reduce drive time and improve case continuity; coverage may extend to the Yolo County/Woodland area as needed
- Schedule is Monday–Friday, 8:30am–5:00pm with all work expected to be completed within the shift
- One weekend shift per month is requested but not required
- Holiday coverage is expected, with flexibility available if time off is requested in advance
- EMR: Homecare Homebase (HCHB) — experience preferred
Requirements
- Minimum 2 years of experience in a healthcare setting required; community or home health experience preferred
- Home health experience including OASIS documentation required
- Active CA PT license required (license required if applicable)
- Current BLS (AHA) certification required
- NPI number required
- Valid driver's license and proof of current automobile insurance required
- Strong oral and written communication skills with good organizational ability
- Ability to work in varied home environments, which may include cramped, hot/cold, or cluttered conditions
- 2 professional references required for consideration: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked (both must include dates of employment and eligibility for rehire)
- Copy of driver's license required for consideration
Additional Information
- PT will supervise up to 2 PTAs or COTAs
- Managed by an RN Manager with support from a Regional Rehab Leader/Educator for therapy-related questions
- Orientation is a robust 3-week process combining online learning and field precepting; solo work is expected by the end of week 2, or week 3 at the latest
- Scrubs are recommended but not provided; business professional dress code applies — closed-toe shoes required, no jeans, tank tops, or logo shirts
- Mileage is reimbursable per GSA guidelines; mileage accrual begins after departure from the first patient assignment, not from the clinician's housing
- Candidates who have been directly employed (full-time, part-time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity, within the past year are not eligible; candidates employed PRN through outside staffing firms will be considered
