Description

American Traveler is seeking a Home Health Physical Therapist with OASIS proficiency and acute care experience for a 13-week contract in Pittsfield, MA.

Details

• Home health setting providing PT services to clients in their place of residence
• Day shift, 8:00 AM – 4:30 PM, 8 hours/day, 40 hours/week
• 13-week contract
• Rotating weekend and holiday coverage required as needed
• No on-call requirement
• Rehabilitation equipment used includes splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities

Requirements

• Minimum 2+ years of acute care (hospital-based) and home health experience required
• Certified home health experience with demonstrated OASIS proficiency required and non-negotiable
• Valid driver's license required; must have personal vehicle available for patient visits
• MA Physical Therapist license required if applicable
• Active AHA BLS certification required
• Home Health Certification required
• Must be able to work independently with minimal supervision
• PT skills checklist current within the past year required for consideration
• One supervisory-signed reference from within the past two years required for consideration
• Candidates must reside more than 50 miles from the facility to be eligible

Additional Information

• Responsibilities include direct patient care, collaboration with interdisciplinary team members, and participation in developing comprehensive patient plans of care
