Description
American Traveler is seeking a Physical Therapist with at least 1 year of home health experience for a 13-week travel contract serving the Big Island of Hawai'i.
Details
- Home health setting serving a primarily geriatric patient population across the island of Hawai'i
- Patient population includes individuals with orthopedic, neurological, cardiac, pulmonary, and cognitive conditions
- Clinical focus includes post-op care (total hip, total knee, spinal surgery), stroke rehabilitation, Parkinson's disease, dementia, COPD, CHF, and peripheral vascular disease
- Productivity requirement of 25 units per week
- Day shift schedule, 5x8-hour shifts (8:00–16:30)
- Uses Homecare Homebase (HCHB) for documentation
- Occasional floating to cover Kauai, Maui, and/or Oahu may be required; all float-related expenses are covered
Requirements
- Minimum 1 year of home health physical therapy experience required
- Current BLS certification (AHA) required
- Active Hawaii PT license required if applicable
- Proficiency in OASIS assessments and Medicare start-of-care processes required
- Experience with geriatric, neurological, and orthopedic patient populations required
- Skills required include evaluation, plan of care development, gait training, adaptive/assistive device use, pain assessment, strength testing, vital signs monitoring, and patient/family education
- Experience with HCHB or similar home health EMR preferred
- Preferred skills include wound care, lymphedema management, MS, spinal cord injury, TBI, and neurodevelopmental technique (NDT)
Additional Information
- Responsibilities include patient evaluation, OASIS completion, plan of care development, short- and long-term goal setting, reassessments, and discharge planning
- Candidate will provide home exercise programs and educate patients and families on safety, injury prevention, medications, and signs of abuse
- Environmental barrier identification and home safety assessment are key components of the role
- First-time travelers are welcome to apply
- Scrubs in any color and closed-toed shoes are required
- Please be prepared to provide total months of start-of-care/OASIS experience and average frequency of Medicare starts of care per week for consideration
