Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of experience and required Homecare Homebase EMR proficiency for a 13-week contract in Charlotte, NC.
Details
- Home health setting providing therapy services to patients in their place of residence
- Typical caseload is approximately 6 visit points per day
- Patient population includes adults and geriatric patients with chronic diseases, wound care needs, IV therapy, PleurX, and wound VAC
- 8-hour day shifts, 8:00 AM – 5:00 PM
- 40 hours per week
- No weekend rotation required
- On-call is not required
- EMR: Homecare Homebase — experience required, no exceptions
Requirements
- Minimum 1 year of home health physical therapy experience required, with most recent experience within the last year
- Active state PT license required; license required if applicable
- Current BLS certification required
- Valid Driver's License and proof of auto insurance required
- Homecare Homebase EMR experience required — no exceptions
- Proficiency with OASIS assessments required
- Experience with IV therapy, PleurX, wound VAC, and wound care required
- Experience in chronic disease management, disease teaching, and maintenance notes required
- Candidate's permanent address must be 50+ miles from the home unit location
- Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the past year
- Last 4 digits of SSN and full date of birth required for consideration

- Responsibilities include skilled assessments, wound care, IV therapy, PleurX and wound VAC management, disease teaching, and OASIS documentation
- Samsung Note 9 phones are used in the field
- Orientation consists of a 5-day NEO (new employee orientation)
- Support provided by departmental leadership
- Grey scrubs required
- Free parking available
- First-time travelers may be considered if they have seasoned home health experience
- Candidates who have worked a permanent position at an AHSE facility must be separated for at least 1 year before being eligible
- RTO requires manager approval
